Support Coordination-
Elodina provides Support Coordination that is structured, proactive, and firmly grounded in participant choice and control. we support participants to implement their NDIS plans, connect with appropriate providers, understand their funding, and monitor how supports are working over time. Our approach focuses on clarity, problem-solving, and capacity building, ensuring participants are informed, supported, and confident in directing their supports- without creating dependence on the system.
Specialist Support Coordination-
Our Specialist Support Coordination service supports participants with complex, high-risk, or rapidly changing circumstances. This includes situations involving multiple service systems, breakdowns in supports, safeguarding concerns, or significant transitions. Elodina works to stabilise situations, reduce risk, coordinate across stakeholders, and develop clear action plans that align with the participant's goals, rights, and responsibilities. This service is delivered with a strong focus on accountability, advocacy, and navigating complexity while maintaining dignity, choice, and control.

Elodina is committed to strengthening community capacity by being present, visible, and actively engaged within the communities we serve. We intentionally position ourselves as part of the community- based activities alongside people living with disabilities- to promote inclusion, understanding, and practical connection. Through presence, education, and shared experience, we support environments where people with disabilities are welcomed, informed, and empowered to participate meaningfully in community life.
Provider Consultancy
Elodina offers Provider Consultancy to NDIS providers seeking deeper understanding of the Support Coordination role or guidance in complex or changing participant situations. This service supports providers who require additional training, clarity around best-practice Support Coordination, or expert consultation when a participant is experiencing significant change and an informed, coordinated response is needed. We work collaboratively with providers to assess the situation, identify risks and priorities, and develop a practical, participant-centered action plan that supports stability, continuity of care, and positive outcomes.
